• The fields must cover standard personal information (full name, email, phone, mailing address) and include an email field that validates format in real time.
• As soon as a guest hits “Submit” or “Send,” they should receive an automatic confirmation so we know the address is active.
• The design needs to match a high-end fashion aesthetic: clean typography, subtle color palette, space for our logo, and a layout that looks just as good on a laptop as it does on a phone screen.
• Final deliverable can be an interactive PDF or an HTML email-friendly form—whichever you recommend for the smoothest email submission and built-in email verification.
